Where the sound comes out
Through AirPods, or any other Bluetooth headphones the Watch is paired with. The Watch speaker is there for calls and alerts, not for a music stream, so a pair of headphones is part of the setup rather than an optional extra. Pair them once and the Watch remembers.
What an hour costs in cellular data
The same as anywhere else, because nothing is added on top of the stream. About 58 MB an hour at 128 kbps, less on a talk station running at a lower bitrate, more on a 320 kbps music station. Macdio shows the bitrate on the station, which matters more on a watch plan than it does on Wi-Fi.
What standalone really needs
The Watch app needs watchOS 11 or newer, and a connection of its own: either a cellular Watch with an active plan, or Wi-Fi the Watch has been joined to before. With either of those the iPhone can stay at home and the radio keeps playing. Without them the Watch falls back to the phone, the same as every other streaming app on the wrist.
Your stations are already on it
Favourites and recently played sync through iCloud, so a station you heart on the Mac is on the Watch by the time you next raise your wrist. There is no pairing step and no separate list to keep up to date, and no account to create for any of it.
What it does not do
Recording, synced lyrics and the full station directory stay on the larger screens. The Watch app is built for one job, which is starting a station you already know and controlling it, and squeezing the rest onto a screen that size would make both halves worse.
